Facility Evaluation Report
Licensing Program Analyst (LPA) Javina George made an unannounced case management health checks visit. LPA was greeted and granted entry by caregiver Evelyn Dallo. LPA met with both Caregivers Evelyn and Monica where LPA explained the purpose of the visit. All three (3) clients were present during LPAs visit, however all (3) clients were observed to be asleep during LPAs visit.
There were no health or safety concerned were observed during the time of the visit, as LPA observed the following:
The facility was observed to be clean and clutter free.
The facility was observed to possess personal hygiene supplies, paper goods and emergency supplies available.
All staff present were observed to have obtained criminal record clearance and to be associated to the facility. The facility utilities such as gas, water and electric were observed to be operable.
Client medications, chemicals and other hazardous items were observed to be locked and inaccessible to clients in care.
The food supply was observed to be adequate, as there was a 2 day supply of perishable and a 7 day supply of non perishable food items.
An exit interview was conducted where this report, LIC811-confidential names list, was reviewed and provided to Evelyn Dallo, Caregiver.
