Facility Evaluation Report
On 1/27/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit to the facility. LPA was greeted by staff Emma Olano and explained to Emma the purpose of the visit. LPA was granted entry and conducted a tour of the facility alongside Emma. During the visit, LPA observed the following:
During the visit, (3) three out of (4) four clients were out of the facility, attending day program. (1) One client stood home due to feeling sick. Indoor and outdoor passageways were observe to be free of obstruction. Client bedrooms were equipped with the required bedding, furniture, and functional lighting. LPA observed a hall closet which stored additional linen and towels. The facility provides its own laundry services. The washer and dryer were observed to be fully operable. LPA observed a fully charged fire extinguisher on the south wing of the facility and another mounted in the kitchen. Night lights were place in the hallways throughout the facility. Kitchen was observed to be clean and free of litter. Facility maintained a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. Water temperature was measured and observed to meet within the required limits. Medication is centrally stored in a locked closet along with client and staff files. LPA observed an additional locked closet which stored cleaning supplies, knives, and other sharp instruments. LPA observed a shaded outdoor area that was fully furnished. Carbon monoxide and smoke detectors were tested and observed to be fully operable. Per staff Emma, there are no firearms and/or ammunition on the premises.
