Facility Evaluation Report
On 1/27/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit. LPA was greeted by staff Felecola "Felly" Cordero and explained to Felly the purpose of the visit. LPA was granted entry into the facility and conducted a tour alongside Felly. LPA was advised that (4) four out of (4) four clients were out of the facility attending day program. LPA observed the following during today's visit:
Indoor and outdoor passageways were free from obstruction. The facility was odor free and free of litter. Clients bedrooms were observed to be equipped with the required bedding, furniture, seating, and functional lighting. Bathrooms were observed to be sanitary and fully operable. The food preparation area in the kitchen was ob serve to be sanitary and organized. LPA observed a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. Knives and other sharp items were maintained in a locked cabinet inaccessible to clients. Medication and client files were stored in locked closet. The fireplace was made inaccessible to clients to ensure protection of the clients' safety. The dual carbon monoxide/smoke detector was tested and observed to be fully operable. LPA observed a fully charged fire extinguisher. LPA observed an covered outdoor patio that was fully furnished. Hot water temperature was tested and tested within regulations. Per Administrator Dionisia Verzosa, there are no firearms and/or ammunition on the premises.
LPA conducted a review of (4) four out of (4) four clients records. Records reviewed included but not limited to admission agreements, preplacement appraisals, reappraisals, medical assessments, TB results, current IPP's, and client personal property and valuables. LPA conducted (2) two staff records review. Staff records reviewed included but not limited to background clearance, fingerprinting, personnel reports, TB results, health screenings, and training verification. During the visit, LPA did not observe any health and safety concerns.
An exit interview was conducted and a copy of this report was provided to Administrator.
