Facility Evaluation Report
On 4/7/26, Licensing Program Analyst (LPA), Kyle Wellington, conducted an unannounced Case Management health and safety check visit and met with Caregiver, John Padilla, and explained the purpose of the visit.
LPA toured the facility with the Caregiver and observed four (4) residents with one (1) staff present at the time of the visit who had clearance and was associated to the facility.
LPA confirmed that the facility has operable utilities (gas, electricity, water) and adequate food supply. There were no immediate medical needs of the residents and no health and safety concerns. The physical plant was observed to be in safe condition. LPA reviewed resident files and obtained admission agreements for the current residents living at the facility.
At the time of visit, LPA spoke with Administrator, Rolando Corpuz, via telephone to inquire about the closure letter that was issued to each resident or their responsible party. The facility failed to provide the residents in care with a 60-day notice. The eviction notice was dated March 30, 2026 with a facility closure date of April 30, 2026.
As a result of the licensees’ failure to comply with the closure plan, the following citation will be cited per Title 22, Division 6 Chapter 8 of the Health and Safety Code.
An exit interview was conducted where a copy of this report along with appeal rights and LIC 809D pages were reviewed and provided to the Administrator Rolando Corpuz.
