The inspector’s account

On 3/26/2026, Licensing Program Manager (LPM), Carolyn Tuba and Licensing Program Analyst (LPA), Kyle Wellington made an unannounced Case Management health and safety check visit. LPM and LPA met with Caregiver, John Rick Padilla and explained the purpose of the visit.

LPM and LPA took a tour of the facility with the Caregiver and observed 5 residents that were present at the time of the visit.

LPM and LPA confirmed that the facility has operable utilities (gas, electricity, water) and adequate food supply. The facility provides 3 meals and snacks between meals and is prepared by the caregiver. During the visit LPM and LPA observed breakfast, lunch and dinner served to the residents. There is sufficient supply of paper and hygiene supplies for the residents in care. Medication is stored in the kitchen in a locked cabinet and separated by each resident. There were no immediate medical needs of the residents.

The physical plant was observed to be in safe condition. LPM and LPA observed an outside area in the back of the house that is separated by a locked gate. According to the caregiver, it is not utilized by the residents, however LPM and LPA observed a substantial number of bees were found swarming under the tool shed, which could pose a potential hazard should the bees increase. A technical violation was issued, and best practice would be to have a pest control check the immediate area for potential beehives.
