Facility Evaluation Report
On 03/19/2026, Licensing Program Analyst (LPA) Jacqueline Shaw-Ross arrived at the facility to conduct the required annual visit. LPA was greeted at the door by Salvacion Bagtas, Caregiver, and the purpose of the visit was explained. Present at the facility were one staff member, one hospice worker, and three residents. The home is approved for six elderly residents ages 60 and above. All of whom may be non-ambulatory, three are approved for bedridden. Hospice waiver is approved for four residents.
The facility is a single story structure with five (5) bedrooms and three and a half (3.5) bathrooms, with one bedroom designated for a live in caregiver.
LPA conducted a tour of the interior and exterior of the facility. The home was observed to be clean, free of clutter, and free of odors. The food supply was observed to be sufficient. Hot water temperature tested in two (2) resident bathrooms measured 113 degrees Fahrenheit, which is within regulatory limits.
Medications, sharps, and cleaning chemicals were locked and inaccessible to residents in care. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Quarterly emergency disaster drills are being conducted as required; the most recent drill occurred on 01/13/2026. No pools, bodies of water, or firearms/ammunition were observed on the premises.
Staff present were observed to have criminal record clearance, facility association, and valid CPR certification. Annual licensing fees are up to date. Emergency drills are conducted quarterly, with the last drill conducted on 01/13/2026. The Emergency Disaster Plan was reviewed and is updated.
During the visit LPA was informed by Caregiver that they did not have access to client records and the owner was away on medical appointment. LPA spoke with the Licensee by telephone and informed them of the importance of having all records readily available for review. Citation issued.
