Facility Evaluation Report

On 11/18/2024, Licensing Program Analyst (LPA) Janette Romero made an unannounced collateral visit at the facility to conduct an interview with Client 1 (C1) in relation to an open complaint against a different licensed facility.

LPA met with Program Manager (PM), Mary Grace Winters who was informed of the purpose of the visit. LPA toured the facility and interviewed C1. During the tour, LPA did not observe any issues or concerns.

An exit interview was conducted where a copy of this report was reviewed and provided to PM Winters.
