On 12/7/2023, Licensing Program Analyst (LPA) Janette Romero made an unannounced collateral visit at the facility to conduct an interview with Client 1 (C1) in relation to complaint control #18-AS-20231101121922.
LPA was greeted and granted entry by Program Manager (PM), Grace Winters who was informed of the purpose of the visit. LPA conducted a tour of the facility with PM Winters and interviewed C1 at the facility.
During the tour, LPA did not observe any issues or concerns.
An exit interview was conducted where a copy of this report was reviewed and provided to PM Winters.
