Facility Evaluation Report

On 07/14/2026, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced case management visit to the facility to deliver an Immediate Exclusion order for Staff Andres Fox on behalf of Community Care Licensing’s San Bruno Regional Office. LPA met with Lead Staff Chris Drozd and explained the purpose of the visit. Administrator Elizabeth "Liz" Gauci was also notified of the purpose of LPA's visit. Drozd reported Staff Fox has not worked in this facility nor is he scheduled to work.

During today's visit, LPA toured the facility's interior and exterior with Drozd and did not observe Staff Fox present in the facility. LPA provided Drozd the Immediate Exclusion order dated 07/10/2026 regarding Staff Fox. During today's visit, LPA did not observe any issues or concerns and no deficiencies were cited. An exit interview was conducted and this report was reviewed and provided to Drozd.
