Facility Evaluation Report

On 2/3/2025, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced visit to the facility to follow up on an alleged incident reported by the facility involving Client 1 (C1) and Staff 1 (S1). Upon learning of the alleged incident, the facility met the reporting requirements in a timely manner. LPA met with Regional Director (PD), Dorothy Williams who was informed of the purpose of the visit. During today's visit there was four (4) clients and four (4) staff present.

LPA toured the facility with RD Williams, conducted interviews, and requested copies of pertinent records. The requested documentation will be provided to LPA by close of business on 2/4/2025. During today's visit, no imminent health or safety concerns were observed. An exit interview was conducted and a copy of this report was reviewed and provided to RD Williams.
