Facility Evaluation Report
On 07/02/25 Licensing Program Analyst (LPA) Javina George conducted an unannounced visit to the facility to conduct a 1 year required visit. LPA met with the Administrator Claudia Manzano Ramos, where LPA explained the purpose of the visit. At the time of the visit there was (4) staff and (1) client present. the other (3) clients were at the day program. The facility is licensed to serve age 18 through 59, 4 non ambulatory only. The facility is approved for delayed egress for the front and east side yard gates.
All staff present were observed to have obtained criminal record clearance and to be associated to the facility. Staff files reviewed were observed to have the required training which includes valid First Aid/CPR certification. The administrator Claudia Manzano Ramos is in possession of a valid administrator certificate that expires on 10/08/25. The facility was observed to be clean, with the passageways being free from obstructions. There are no pools or bodies of water on the premises. The facility food supply was observed to be adequate and the hot water was tested and found within regulatory limits measuring at 117.8 degrees Fahrenheit. The facility is required to have a sprinkler system and the last inspection was conducted in January 2024. The fire extinguishers were fully charged and last serviced in April 2025. The facility is conducting emergency disaster drills on a monthly basis and the last disaster drill was on 06/08/25.
The medications stored in a medication cart inside the locked medication room Records review the clients files were observed to have the required documents such as Admissions agreement, and medical assessment. . The personal incidental (P&I) was reviewed for all (4) clients and there were no discrepancies found. There were no deficiencies observed during today’s visit.
An exit interview was conducted where a copy of this report, LIC811-Confidential names list was reviewed and provided to Claudia Manzano Ramos, administrator.
