The inspector’s account

On 01/23/2026 Licensing Program Analyst (LPA) Jacqueline Shaw-Ross made an unannounced visit to the facility to conduct an annual inspection. LPA was greeted and granted entry by Administrator Elvira Sison, where LPA explained the purpose of the visit. The facility is licensed to serve six (6) elderly residents, ages 60 and above. Five (5) of whom may be non ambulatory and one (1) bedridden, and resident in room #3. The facility has an approved hospice waiver for (6). There are currently (4) residents receiving hospice services. Below is a summary of what was observed during today’s inspection:

Infection Control: LPA observed that the facility has an Infection Control Plan on file that was approved on 2/15/22. Facility staff were observed to demonstrate best practices in the facility to maintain a healthy environment for staff and residents such as proper hand hygiene and wearing masks. The facility is also taking the resident temperatures daily.

Physical Plant: LPA toured the interior and exterior of per staff there are no firearms or dangerous weapons on the premises; no bodies of water were observed; smoke detector and carbon monoxide were operable; hot water temperature measured at 112; grab bars for each toilet, bathtub and shower; non skid mats or strips in shower or bathtub, and there were sufficient hygiene supplies. The outdoor walkways are unobstructed and there is a shaded sitting area available for resident use. LPA observed that there a sufficient bedrooms and bathrooms for both staff and residents. The exits are not obstructed and that there is plenty of space for activities.

Records Review: Staff Records: LPA observed that there are sufficient staff present (3) to meet the needs of residents. LPA additionally confirmed that there is an Administrator present with a valid certificate which expires on 07/13/26 for Administrator Elvira Sison. LPA confirmed staff have criminal record clearance and were associated to the facility and have training to perform their required duties.
