The inspector’s account

On 5/5/25 Licensing Program Analyst (LPA) Javina George made an unannounced 1 year required visit. LPA was granted entry and met with Administrator, Kathrin Baer and Operations Manager Uriel Neis who was informed of the purpose of the visit. At the time of the visit there was (7) staff and (18) clients present. The facility is licensed to serve clients served are between the ages of 18-59 who have been diagnosed with a neurological handicap, of which all may be non ambulatory.

The facility is a single story structure consisting of with (10) bedrooms and (5) bathrooms. The facility is comprised of two (2) buildings that are adjacent to one another and housed on the same lot as the day program (372008307). There are no pools, bodies of water, or known guns being kept at the facility. The facility consists of TV room, staff office, Laundry room and lounge area, in addition to the dining area as well as the kitchen. The facility has an added therapeutic tool, as a therapy dog has been contracted. Therapeutic services are being offered Tuesday-Friday.

During today's visit LPA randomly tested the pull cords in client bedrooms and observed for them to be operable, as well as staff responding within 2 minutes. The facility is equipped for fully charged fire extinguishers. The food supply met with minimum of a 2 day supply of perishables and a 7 day supply of non perishable food items. LPA observed for there to be expired food items consisting of (1) box of pudding and (4) cans of enchilada sauce. The items were discarded at time of the visit therefore no citations were issued. The facility was observed to be clean and the passageways were from any obstructions. The hot water measured to be be at 119.8 degrees Fahrenheit. The medications are stored in a locked medication cart inside the locked medication room. The facility is conducting emergency disaster drills on a quarterly basis, the last drill conducted was on 03/27/25. The facility annual fees that were due on or before 5/23/25 were paid during today's visit.
