Facility Evaluation Report

On 2/18/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced case management visit to follow up with a personal rights allegation reported by the facility regarding an alleged incident on 2/7/2025. LPA met with Program Director, Bill McAweeney and Administrator, Katherine Baer who were informed of the purpose of the visit.

LPA toured the facility and obtained copies of pertinent records. During today's visit, no health or safety concerns were observed and no deficiencies were cited. An exit interview was conducted and a copy of this report was reviewed and provided to PD McAweeney.
