Facility Evaluation Report
Licensing Program Analyst (LPA) Javina George conducted an unannounced annual required visit. LPA was granted entry and met with Uriel Neis, Operations Manager and Program Director Bill McAweeney who was informed of the purpose of the visit. LPA was informed that the current administrator Kathrin Baer was out of the office today.
At the time of the visit there were zero (0) clients present as they were out at a beach outing. The facility is licensed to serve neurologically handicapped head injured adults, aged 18-59, all of whom may be non-ambulatory.
The facility is a story facility that offers day treatment program that consists of the following therapies: speech, occupational, physical and recreational, music and neuro-psychological and individual and group counseling. The facility has activity room, music room, gym, multiple staff offices, break room, and a shaded area to promote and encourage socialization.
Operating Requirements: The facility has an approved fire clearance that is for 28 non-ambulatory clients combined with the residential program next door. The last fire inspection was completed on 11/20/23. The facility was observed to have operable smoke and carbon monoxide detectors. The facility ratio is 3 clients to 1 staff.
Record Reviews: LPA conducted a review of the current roster compared to the staff currently at the facility and all were observed to have obtained a criminal clearance and were associated to the facility. The next upcoming CPR/First aid training is being held at the beginning of April and Crisis Prevention Intervention (CPI) training is being held at the end of April 2024. LPA observed for staff to have a current CPR/First Aid Certification. Staff and resident files reviewed were found to be complete.
