Facility Evaluation Report
On 04/15/2026, Licensing Program Analyst (LPA) Jacqueline Shaw-Ross arrived at the facility unannounced to conduct a required annual visit. Upon arrival, LPA observed that the facility appeared vacant. LPA phoned Administrator, Garrett Welker and who stated that the facility closed down effective 04/01/2026. Administrator reported the reason for closure was initiated by the owners and was due to constant repairs needed to the physical plant of the home. LPA was informed three residents went to three of their other facilities, and the other three residents moved to other care home. LPA toured the facility and confirmed that no residents were occupying the facility. The effective date of the license was 05/18/2021 and Administrator surrendered the original copy of the facility license during the visit. LPA explained to Administrator that the license would no longer be valid and therefore no required care and supervision should be provided in the home unless the state approves Licensure in the future.
