The inspector’s account

On 07/31/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required inspection. LPA met with Administrator Marilou Manrique, where LPA explained the purpose of the visit, At the time of the visit there was (2) staff and zero (0) clients present as (3) clients were at the day program and (1) client was at an appointment. Below are the observations made during today's visit. LPA verified facility contact information, and observed to have valid liability insurance as well as a surety bond.

The facility was observed to be clean, and clutter free. The food supply was adequate and the water temperature was within regulatory limits (105 degrees Fahrenheit). The emergency disaster drills are being conducted on a monthly basis, with the last drill being conducted on 07/09/25. The facility is equipped with solar panels and is in possession of emergency supplies. The fire extinguishers are fully charged, with the tag in tact. There are no guns or bodies of water on the premises. The medications, sharps and chemicals were observed to be locked in inaccessible to clients in care. The smoke and carbon monoxide detectors were tested and found to be operable.

All staff present were had criminal record clearance and were associated to the facility. The staff possessed Cardio Pulmonary Resuscitation training, and the administrator has a valid administrator certification that expires 11/28/25. Client files had admission's agreements, medical assessments and Individual Program Plan (IPP). Client Personal and Incidental (P&I) funds were reviewed with no discrepancies found. Based on today's inspection no deficiencies were observed.

An exit interview was conducted where a copy of this report was reviewed and provided to Marilou Manrique, administrator.
