Facility Evaluation Report
On 07/13/2026, Licensing Program Analyst (LPA) Aziz Faizi made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Administrator Rhea Tabler who was informed of the purpose of the visit.
The facility is a one-story building consisting of nine (9) activity rooms, three (3) bathrooms, a kitchen and a staff lounge. There are no pools or known firearms on the premises.
The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The facility does not provide meals services to clients. Snacks available to clients were observed to be in within expiration dates.
LPA observed adequate hygiene supplies and cleaning materials stored in the janitor closet and in the garage laundry area. Although the facility does not have an outdoor space, the garage is utilized for client activities and was free of hazards. Sharp and dangerous items in the kitchen were locked and stored inaccessible to clients.
Restrooms were clean and had toilet paper, paper towels, and soap readily available for clients. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 105.2°F.
LPA observed three (3) fire extinguishers throughout the facility to be in compliance with the department's requirements with the expiration date of 02-17-27.
