Facility Evaluation Report
On 11/18/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required annual inspection. LPA met with Administrator,Flor De Lys Barawid, and explained to Lys the purpose of the visit. During the inspection, (5) five out of (5) five clients were out in the community, attending day program. LPA Flores was granted entry and conducted a tour of the interiors/exteriors areas of the facility. LPA Flores observed the following during the visit:
The facility is a single-story structure which consists of (4) four client bedrooms, (2) staff bedrooms, (3) three bathrooms, living room, activity room, formal dining room, kitchen, and garage. The facility maintained a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. Facility stores knives and other sharp items in the locked staff bedrooms. LPA observed emergency food bins and water to meet clients needs in case of an emergency. Client bedrooms met the required bedding, seating, furniture, and functional lighting. Indoor/outdoor passageways were free from obstructions. LPA observed activities in the activity room to promote group socialization. The medications are stored in a locked cabinet in the kitchen that was made inaccessible to clients. Smoke and carbon monoxide detectors were tested and were observed to be fully operable. Facility provided laundry services and LPA observed detergents and other cleaning solutions in a locked cabinet. LPA observe outdoor seating with sufficient shading. Per Administrator Lys, there are no firearms and/or ammunition on the premises.
LPA observed the See Something Say Something, Client Personal Rights, facility sketch, emergency disaster plan, and license posted on the wall near the dining room.
