Facility Evaluation Report
On 6/23/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver, Gabriella Gallegos who was informed of the purpose of the visit. LPA met with Caregiver, Maria Cristina Gabriel who contacted Administrator, Justine Silverio to inform them of the purpose of LPA's visit. The facility has a fire clearance to serve four (4) non-ambulatory clients and an approved hospice waiver for two (2).
LPA conducted a tour with Gabriel and observed the facility is made up of a one-story home with three (3) client bedrooms, two (2) bathrooms, a kitchen, dining area, living room, laundry room, and attached garage. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had grab bars and non-skid mats in the showers. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility has a two-day supply of perishable foods and seven-day supply of non-perishable food items. Medications are secured in a locked cabinet near the dining room. Cleaning solutions and disinfectants are secured in a locked hallway closet. Gabriel tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard them to be operational. LPA also observed charged fire extinguishers mounted in the facility that were last serviced on 6/10/2025. The facility's certificate of liability insurance expires on 6/28/2026. Staf f present have a criminal record clearance and are associated with the facility. Client files reviewed had Departmental required records. The facility was reminded their annual licensing fees are on due on 6/29/2025 and administrator reported they will be paid by the due date . L PA reviewed the facility's "Fire/Earthquake Drills" log noting the facility's last fire and earthquake drill was conducted on 6/1/2025. Emergency phone n umbers, complaint procedures, clients' personal rights, and facility sketch are visibly posted throughout the facility. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Caregiver Gabriel.
