Facility Evaluation Report
On 9/10/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required annual visit. LPA met with care staff, Rosa Flores, and explained the purpose of the visit. LPA conducted a tour of the facility and did not observe any health and safety concerns. During the time of visit, (2) two out of the (3) three clients were attending day program. The (1) one client in the facility was sleeping during the time of LPA's visit. During today's visit, LPA Flores observed the following:
The physical plant is a single story structure which consist of a formal dining room, kitchen, garage, (3) three client bedrooms, (3) three staff bedrooms, and (2) two bathrooms. Client bedrooms were equipped with the required bedding, furniture, and functional lighting. All bathrooms were in safe and sanitary operating conditions. The facility operates their own laundry services; washer and dryer were observed to be in good repair and operable. Additional linen and towels are available for clients and appear to be in good repair. LPA did not observe no pools or bodies of water. The facility met the (2) two-day supply of perishable food and (7) seven-day supply of non-perishable items. Per Administrator, Melissa Sumibcay, there are no firearms or ammunition on the premises.
Indoor/outdoor passageways were free of obstructions. Outdoor patio area was observed to have shading and sufficient seating. LPA observed knives and other sharp instruments locked in a cabinet located under the kitchen skin. Water temperature measured at 111.5-degree Fahrenheit meeting within the department's requirements.
