Facility Evaluation Report
On 10/8/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required annual visit. LPA was greeted by Administrator, Melissa Sumibcay, and was granted entry. LPA advised Administrator of the purpose of the visit. A tour of the facility was conducted. LPA Flores observed the following during todays visit:
During the visit, there was (1) one client present; all other clients were attending day program. The physical plant is a two-story structure which consist of a dining room, kitchen, garage, (5) five client bedrooms, (2) two staff bedrooms, and (3) three bathrooms. Client bedrooms were equipped with the required bedding, furniture, seating, and functional lighting. All bathrooms were in safe and sanitary operating conditions. Additional linen and towels are available for clients and appear to be in good repair. LPA observe any empty pools which is locked at all times with fencing that is more than 5 ft. high around the perimeter of the pool. The facility met the (2) two-day supply of perishable food and (7) seven-day supply of non-perishable foods. LPA observed knives and other sharp instruments locked in a cabinet located in the kitchen. Indoor/outdoor passageways were free of obstructions. Carbon monoxide and smoke alarm was tested and appeared to be operable. Centrally stored medication was placed in a locked cabinet. Per Administrator, Melissa Sumibcay, there are no firearms or ammunition on the premises.
Facility sketch, client's personal rights, and complaint information are visibly posted near the front entrance. Client records included but not limited to signed admission agreements, physician reports, weight charts, and valid IPP's. During today's visit, LPA did not observe any issues or concerns.
An exit interview was conducted and a copy of this report was reviewed and provided to Administrator, Melissa Sumibcay.
