Facility Evaluation Report
Licensing Program Analysts, (LPA) Armando Perez and Abdoulaye Zerbo made an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA met with Applicants Sarai Marcelin and Lorena Maharaj who accompanied LPAs for the inspection. The application is for Adult Day Program. On August 12, 2024, City of Escondido Fire Department approved a fire clearance for the facility and has been granted for (40) Ambulatory and (20) non-ambulatory clients. The Day Program is located in a commercial building.
Facility is one level building with 8 common rooms with each room having a different activity and 7 designated rooms for staff. Staff rooms include administrator offices, a staff lounge, a kitchen, a laundry room, a quiet room and medication room. LPA observed an adequate supplies of reading materials, games, equipments and other activity materials.
Facility has two restrooms with one designated for male clients and the other for female clients. Each restroom has the capacity for five clients at a time. Clients' bathrooms were observed to be clean, and the toilets were found to be working order. The facility's water temperature measured at 112.9 degrees Fahrenheit. LPA observed a laundry room were hazard and/or toxic material will be stored and secured. There is also a secure medication room for medication storage. Facility has four fire extinguishers that were inspected and found to be in compliance. There are no bodies of water observed near and/or on the premises. The facility does not have firearm and/or ammunition on grounds.
Licensee will be providing snacks to clients during program hours. There were sufficient amount of 7 day non-perishable snack items observed. However, there no 2 day perishable food supply observed as clients have not been admitted to the facility yet and facility will not prepare meals for the clients. Administrators informed LPAs that clients will be providing their own lunches. LPAs observed the required postings of the emergency disaster plan, resident personal rights, complaint procedures, employee rights, visitation rights, facility sketch, and the Long-Term Care Ombudsman poster. Facility contains emergency supplies and first aid kits with the required items. The facility has working telephone for client use. LPA observed Applicant's Administrator Certification to be in pending status and a valid CPR certification on file with expiration date of 03/17/2025
