The inspector’s account

On 04/08/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and granted entry by Administrator Arni Montazer where LPA explained the purpose of the visit. (6) non ambulatory residents of which 1 may be bedridden. At the time of the visit there were (5) residents present of which 4 were asleep during LPAs visit.

The facility is licensed to serve age range 60 and over and an approved hospice waiver for (6) with (1) resident currently receiving hospice services. During today's visit LPA verified contact information and will update accordingly. The facility annual fees that were due by 4/4/25 were observed to be paid, and the governing body was observed to be active and in good standing.

The facility interior and exterior areas of the facility were observed to be clean, and free of any obstructions. The food supply was observed to be sufficient as the requirement was met. The facility has a clean pool that is housed behind a locked fence. There are no known guns or ammunition. The emergency disaster drills are being conducted on a quarterly basis with the last drill being on 3/2/25. The fire extinguishers are fully charged and were last serviced 8/5/24. The hot water measured to be within regulatory limits at 113 degrees Fahrenheit. The medications, sharps chemicals and other hazardous items were observed to be locked and in accessible to residents in care. The facility is armed with a security alarm and video surveillance.

All staff present had the required training and possessed valid CPR certification. Staff have criminal record clearance and are associated to the facility. The Administrator Arni Montazer's administrator certification expires on 8/12/26. Resident files had the required documents such as admissions agreement, medical assessment and appraisals. Based on today's inspection no citations were issued. An exit interview was conducted and a copy of this report was reviewed and provided to Arni Montazer, Administrator.
