Facility Evaluation Report
Licensing Program Analyst (LPA) Jacqueline Shaw-Ross conducted an unannounced visit to the facility to complete a one-year required inspection. Upon arrival, LPA was greeted by Administrator Dearme Doverte and granted entry. LPA explained the purpose of the visit. At the time of inspection, there were two staff members and four residents present. The facility has an approved hospice waiver for five residents. Currently there are no residents receiving hospice services.
The facility is a one story four bedroom two bathroom home. A tour of the facility was conducted both inside and outside. Furnishings were in good condition in the living room, bedrooms, and dining room. The home appeared clean and odor-free. Medications were observed to be locked in a cabinet located in the kitchen between the refrigerator and the double ovens. No pools or other bodies of water were observed on the premises. There are no known firearms or ammunition stored on site. All sharps were observed to be locked and inaccessible to residents.
Emergency drills are conducted monthly, with the last drill completed on 01/05/2026 . Hot water temperatures in resident bathrooms were tested and found to be within regulatory range at 113 degrees Fahrenheit. The governing body remains active and in good standing, and annual fees have been paid.
