Facility Evaluation Report
On 9/29/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required annual visit. LPA was greeted by Administrator, Makiz Asatyar, and was granted entry. LPA advised Administrator of the purpose of the visit. A tour of the facility was conducted. LPA Flores observed the following during todays visit:
The facility is a two story structure which consist of (3) three client bedrooms, (4) four bathroom, (1) one staff office, (1) one staff bedroom, formal dining room, living room, kitchen, garage, and backyard. During the visit, (3) three out of (3) three clients were out in the community. LPA observed indoor/outdoor passageways to be free of obstruction. The facility was free of insects, rodents, and vermin. LPA observed a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. LPA observed a locked cabinet designated for knives and other sharp materials. Client bedrooms were equipped with the required bedding, furniture, seating, and functional lighting. Additional linen and towels are available in a hallway closet for clients and appear to be in good repair. LPA observed books, games, and other activities located on a bookshelf on the lower level of the facility. Smoke and carbon monoxide detectors were deem fully operable. LPA observed a fully charged fire extinguisher mounted on the wall in the kitchen. The facility has a pool which is locked at all times with fencing that is more than 5 ft. high around the perimeter of the pool. LPA observed a fully stocked first-aid kit. Per Administrator Makiz, there are no firearms and/or ammunition on the premises.
