The inspector’s account

On 12/26/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver, Ricardo Honorio who was informed of the purpose of the visit. LPA called Administrator Robert Jazmin and also informed him of the purpose of LPA's visit. Then he arrived to the facility to meet with LPA. The facility has a fire clearance to serve four (4) non-ambulatory adults ages 18-59. During today's visit, there was one (1) staff and one (1) client present and LPA was informed the remaining two (2) clients were out in the community.

LPA toured the facility with Caregiver Honorio and Administrator Jazmond and observed the facility is made up of a one-story home with three (3) client bedrooms, two (2) bathrooms, a kitchen, dining area, and living room. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had grab bars and non-skid mats in the showers. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility had a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are secured in a locked kitchen closet. Administrator tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen that last serviced on 08/20/2025. Staff present has a criminal record clearance and is associated with the facility. Client files reviewed had updated physician's reports and all the Department's required records.
