Interview with resident family member on November 18, 2022 revealed their family member was admitted to the facility in January 2020 and it was known that the Adult Day Program was closed at that time.

Interview with new Executive Director (ED) on November 18, 2022 revealed the Adult Day Program (ADP) re-opened on October 2022.

Records review revealed that from January 2022 through October 2022 the ADP did not have any clients listed on their roster.

Based on LPA observations, interviews, and facility records, we have found that the preponderance of the evidence standard has not been met, therefore, the allegation is found to be unsubstantiated.

An exit interview was conducted with Richard Mariona_and a copy of this report and Licensee/Appeal Rights (LIC 9058) were provided to Richard Mariona whose signature below confirms receipt of documents.
