Facility Evaluation Report
Licensing Program Analyst (LPA) Ramin hashemi conducted an unannounced case management visit to conduct follow up regarding Incident reports received by Community Care Licensing. LPA was greeted by, identified themselves to, and explained the purpose of the visit with J oe Vasquez MSN, RN Sr. Director of Clinical Education.
On June 12th, 2026, the Department received incident reports dated on June 10th and June 12th that the same resident had fallen multiple times with serious injuries.
During today’s visit, LPA conducted a health and safety check, observed residents in care, reviewed facility records, interviewed residents and staff.
No deficiencies were cited on today’s date. An exit interview was conducted with Joe Vasquez MSN, RN Sr. Director of Clinical Education , whose signature below confirms receipt of a copy of this report and the Licensee Appeal Rights (LIC9058 3/22).
