Licensing Program Analyst (LPA) Rebecca Borunda conducted an unannounced Collateral visit. LPA was greeted by, identified herself to, and discussed the purpose of the visit with Administrator Wendy Engle.
During today's visit, LPA interviewed a client for a complaint investigation filed against another facility licensed by Community Care Licensing.
An exit interview was conducted with Administrator Wendy Engle, whose signature below confirms receipt of a copy of this report and the Licensee Appeal Rights (LIC9058 3/22).
