Facility Evaluation Report
Licensing Program Analyst’s (LPAs) Carmen Lopez and Ryan Fulton conducted an announced case management visit for a Pre-Licensing inspection at an Adult Residential Facility for a change of location on today's date to inspect the facility for compliance. After arriving at the facility, LPA’s identified themselves and disclosed the purpose of the visit with Loretta Talia, Licensee, and Shevell Sterling, Administrator. LPA’s were granted entry by Shevell Sterling, Administrator.
During today’s inspection LPAs toured the physical plant, and observed the following client accommodations that will be brought with the clients from the facility along with the furnishings, linens, and personal hygiene items; resident bathrooms will be equipped with non-skid flooring.
Medications are currently stored in a locked cabinet located in the garage but will be stored in a locked cabinet located in the kitchen area of the new facility. Sharps are currently locked in a cabinet located in the garage and stored in a locked cabinet located in the kitchen area. Staff and resident records are kept in a secured cabinet in the garage and will be stored in the locked area with the medications; Food service including dishes, utensils, food storage, and a seven (7) day supply of non-perishables; the two (2) day supply of perishables will be brought over from this facility once required approval is met.
Toxic substances are currently stored in a locked cabinet in the garage and will be stored in a locked area in the next facility; first aid kit and first aid manual were present and will be taken with them upon the licensure of their new facility. It will be stored in a locked island of the facility.
Fire extinguisher (01) was present and serviced within the last 12 months; required facility postings were present and visible in the common areas of the facility and will be taken with them and posted once their new facility has been licensed. According to the Administrator Sterling, there are no guns, weapons, or ammunition stored on the facility property and none will be stored in their new facility.
