Facility Evaluation Report
Licensing Program Analyst (LPA) Carmen Lopez made an unannounced visit to the facility to conduct an annual required licensing inspection and provided facility with additional guidance. LPA identified herself and was granted entry by Roosevelt Edwards, Facility Manager. LPA met with Shevell Sterling, Administrator and Facility Manager Edwards and discussed the purpose of today’s visit.
During today’s visit, September 16, 2022, LPA witnessed an AWOL. While at the facility LPA toured the facility, spoke with staff and client, and requested and obtained relevant client documents for Client #1 (C1 – See LIC 811 Confidential Names list). The facility will report that on September 16, 2022, C1 eloped from the facility but Administrator returned the client to the facility. Facility will report and submit an LIC624 for today’s incident. No injuries were observed to C1.
LPA provided guidance to Facility Manager Edwards on Title 22, Division 6, Chapter 8, Section 81061 Reporting Requirements; and Records to be Maintained at the Facility – Adult Residential (LIC311C), and provided a copy of the LIC311C.
This case management needs further follow-up. No deficiencies were cited during this visit.
An exit interview was conducted with Admiistrator Sterling. A copy of this report, along with the Licensee Rights (01/2016) was provided to Administrator Sterling at the conclusion of the visit. The signature below serves as confirmation of receipt of these documents.
