The inspector’s account

Licensing Program Analyst (LPA) Iby Strong conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Caregiver Lakeisha Woods. Shevelle Sterling arrived shortly after. According to the facility’s license, the facility has a maximum capacity of four clients, all whom must be ambulatory.

LPA toured the interior and exterior of the facility and inspected each room. Client bedrooms contained the required furnishings. Doors, windows, toilets, and showers were in working order. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. Water temperature was measured at 95 degrees F in the kitchen and 85 degrees in bathroom. Outdoor space was not clean, with undone hose, plastic trees on the ground, additionally there was trash on the ground and construction debris. LPA observed cleaning solutions in the client bathrooms, unlocked and accessible.

Cooking/dining equipment and utensils were present. Medication was locked and labeled. Medication administration log was complete.

A pool is present with a locked gate. Per Administrator, no firearms or ammunition are kept at the facility. Carbon monoxide detector was replaced on the spot. Emergency lighting, and facility telephone were all working. Fire extinguisher was present. First aid kit were complete and readily accessible. Fireplace was accessible to clients. Client records reviewed had required documentation.

Two technical violations were issued today for water temperature and fireplace cover. Two technical advisories were issued for backyard cleanliness and obstructions. One type B deficiency was issued for unlocked cleaning supplies.

An exit interview was conducted with Administrator to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22), LIC809 D were provided during the visit.
