Facility Evaluation Report

Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced Case Management Visit due to a request to add one (1) non-ambulatory bedroom. LPA was granted entry after identifying herself and disclosing the purpose of the visit. Administrator Ashlee Menefee later joined the visit.

LPA toured the facility and inspected the room pertinent to the request. LPA observed that the room matched the approved bedridden clearance that was granted by the local fire authority on 3/26/2025. The facility is approved for one (1) bedroom to serve a non-ambulatory client.The request did not involve changing the facility's total capacity, which remains the same at six (6) clients.

No health or safety issues were observed and no deficiencies were cited during this visit.

An exit interview was conducted with Assisted Living Director Michelle England, who was provided with a copy

of this report and Appeal Rights (LIC9056 03/22). Their signature confirms receipt of these documents.
