Facility Evaluation Report
Licensing Program Analyst (LPA) Iby Strong conducted an unannounced visit for the Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Program Director Mark Fisher.
LPA, accompanied by Mark Fisher and Health & Safety Manager Kim Holt, toured the interior and exterior of the facility, and inspected multiple rooms. The facility was clean, sanitary and in good repair. Pathways were free of obstruction and slip hazards. The rooms contained the required furnishings. Doors, windows, toilets, and showers were in working order. Shower water temperature was measured at 107 degrees F. The facility had sufficient space and equipment to facilitate programs, meetings, activities, and dining. LPA observed appropriate levels of staffing to clients in care.
Cooking/dining equipment and utensils were present, and all safely stored. Kitchen water temperature was measured at 133 degrees with a hot water sign present. Food was observed to be stored in a safe manner. There were no toxic chemicals/poisons accessible to clients. Medications were properly labeled, as required, and stored in locked areas. Medication administration records were complete.
There is a pool present and actively being used with multiple staff and life guards present. Fencing and locks were present. Fire extinguishers were present and serviced within the last 12 months. First aid kits were complete and readily accessible. Emergency disaster drill documentation was reviewed.
LPA Strong reviewed 10 staff records and 10 client records, all were complete and locked in secured areas.
