Facility Evaluation Report

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced collateral visit to the day program. LPA Lopez identified herself and was granted entry by Jose Barraza, Receptionist. LPA stated the purpose of the visit with receptionist Barraza.

During the visit, LPA Lopez was unable to speak with clients to aid in a case involving a different licensed care facility. The clients were on their transport ready to be transported back to their respective facility. Receptionist Barraza provided LPA a copy of the clients Case Manager contact information. No deficiencies were observed during today's visit.

An exit interview was conducted, and a copy of this report along with Licensee/Appeal Rights (LIC9058 03/22) were provided to receptionist, Jose Barraza, at the conclusion of the visit. The signature below confirms the receipt of the documents.
