Facility Evaluation Report

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced collateral visit to the Day Program. LPA Lopez identified herself and was granted entry by Wendy Martinez and Jose Barraza, concierge. LPA stated the purpose of the visit with Zulma Garcia, Assistant Program Director.

During the visit, LPA Lopez spoke with clients to aid in an open investigation involving a different licensed care facility. No deficiencies were observed or cited during today's visit.

An exit interview was conducted, and a copy of this report along with Licensee/Appeal Rights (LIC9058 03/22) were provided to Zulma Garcia, Assistant Program Director, at the conclusion of the visit. The signature below confirms the receipt of the documents.
