Facility Evaluation Report
Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced Required 1 - Year Visit. LPA was greeted by, Program Manager, Erin Perschbacher to whom she identified herself and discussed the purpose of the visit. It was noted that four (4) of five (5) staff present at the facility had a current criminal record clearance; however, they were not associated with this facility.
The facility is licensed to serve up to 50 clients ages 18 and over of which six (6) may be non-ambulatory. During today’s visit there were 49 clients in care.
Physical Environment: The facility was clean and free from safety hazards. Adequate lighting and ventilation were observed in all areas. All necessary safety equipment, such as fire extinguishers and emergency exits, was present and in good working condition.
Staffing and Training: The facility had a sufficient number of qualified staff members to meet the needs of the residents. The staff member was observed to be professional, courteous, and knowledgeable in their respective roles. All staff members had completed the required training and certifications per the licensing regulations. Staffing schedules were posted and adhered to, ensuring adequate coverage at all times.
Resident Care and Services: Residents' care plans were reviewed and found to be comprehensive and up-to-date. Medication administration was observed to be in accordance with the facility's policies and procedures. Recreational activities and social engagement opportunities were available.
