Staff did not keep facility free of drug paraphernalia – Unsubstantiated
RP alleges that the facility has drugs on the premises of the facility, and that the facility does nothing to stop it. On 12/6/2023, LPA Carmen Lopez interviewed staff (S1-S4) and stated the facility does not tolerate drug paraphernalia at the facility and residents are using drugs outside of the facility when they out in the community. When staff finds that residents have drug paraphernalia, staff would confiscate the drugs which then would be destroyed. Staff stated no residents have been caught using drugs on the premises. On 7/17/2026, LPA Andrew Christy interviewed staff (S1, S2, S5) and stated the facility’s policy is that residents cannot use drugs on the premises of the facility, and residents signed a form which has the house rules. LPA Andrew Christy interviewed residents (R6, R7) and residents stated they do not use drugs and have not observed residents using drugs in the facility.
Staff did not prevent a client from being physically injured while in care – Unsubstantiated
RP alleges a resident (R1) feels unsafe around other residents, stating that the other residents are “violent offenders, sex offenders, or hard-earned criminals or crooks”. During an interview with R1, R1 states that they have not had any incidents with residents. In addition, interviews conducted with staff (S1-S4) on 12/4/2023, staff stated there have been no instances of physical altercations between residents. Interviews with residents (R2-R6) confirm there are arguments between residents; however, there were no physical altercations between residents.
LPA has fully conducted an investigation for the above allegations. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did occur, therefore the allegations are UNSUBSTANTIATED.
