Licensing Program Analyst (LPA) Renita Hall conducted an unannounced case management visit to the temporary location 435 West Main Street, El Cajon, CA LPA met with Rubiela Aguiar, Licensee, and discussed the purpose of the visit.

A health and safety check was conducted to ensure the appropriate temporary relocation of the resident during fumigation (tenting) of the facility. All residents have been safely relocated to an approved temporary location as of May 28, 2025, with necessary services and supports in place to ensure continuity of care.

The home is scheduled to be cleared for re-entry on Friday, May 30, 2025, at which time all residents will be returned to the licensed facility. The licensee has provided a detailed relocation plan, along with contact information for the temporary site. No immediate health or safety concerns were observed during the check.

The licensee is to notify the Licensing Program Analyst once residents have returned to the facility. An exit interview was conducted with Rubiela Aguiar, Licensee, to whom a copy of this report, along with Licensee/Appeal Rights (LIC 9058 01/16), was provided after the visit.
