Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced collateral visit to the day program. LPA identified herself, stated the purpose of the visit and was granted entry by Cindy Pedrero, manager .
During the visit, LPA spoke with a client to aid in a case involving a different licensed care facility. No deficiencies were observed or cited during today's visit.
An exit interview was conducted with Cindy Pedrero, manager, and a copy of this report along with Licensee/Appeal Rights (LIC9058 03/22) were provided at the conclusion of the visit. The signature below confirms the receipt of the documents.
