Facility Evaluation Report

Licensing Program Analyst (LPA) Liliana Silveira conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Manager Cindy Pedrero.

Today's visit was in response to a Report of Suspected Dependent Adult/Elder Abuse (SOC 341), which Licensee self-submitted to Community Care Licensing Division on 06/17/2025. According to the report, on 06/11/2025, facility staff were made aware that Staff #1 (S1) engaged in inappropriate behavior with Client #1 (C1). [See LIC 811 Confidential Names List for a description of C1]. S1's last day of work at the facility was 06/10/25 and staff was immediately terminated upon discovery of the behavior.

During today’s visit, LPA performed a facility tour / welfare check, collected records, and interviewed staff and C1.

No deficiencies were cited during today's visit. An exit interview was conducted with Cindy, to whom a copy of this report the LIC811 Confidential Names List, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.

NOTE: LPA left the facility for a one hour lunch break and returned to complete the case management visit.
