Facility Evaluation Report

Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced visit to the facility to conduct a Plan of Correction (POC) visit to confirm that a citation, which was issued on 08-29-2024, has been corrected. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Administrator Nicole Guibert.

The following citations were reviewed during today's visit:

82088(e)(1) Fixtures, Furniture, Equipment, and Supplies : Since 08-29-2024, Licensee made temperature adjustments to the day program facility's boiler. During today's visit, LPA retested the hot water at all taps used by clients, and all were compliant: Public Restroom #1 Sink was 108.7 F, Public Restroom #2 Sink was 117.5 F, Common Area Sink #1 was 117.5 F, and Common Area Sink #2 was 117.7 F. The Plan of Correction is Satisfied.

No new deficiencies were identified or cited during today's visit.

An exit interview was conducted with Guibert, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
