The inspector’s account

Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced case management visit. LPA was greeted by and allowed entry into the facility by Administrator Kevin Fleck and discussed the purpose of the visit regarding an increase in capacity.

The Licensee applied for an increase in total capacity from six (6) residents to nine (9) residents. The approved Fire Clearance dated 11/25/25 reflects a total capacity of nine (9) residents in care, seven (7) may be non-ambulatory and two (2) may be bedridden. LPA toured the facility and observed it to reflect the updated Fire Clearance and floor plan.

No deficiencies were observed or cited during today's today.

An exit interview was conducted with Administrator Kevin Fleck and a copy of this report and the Licensee Rights (LIC 9058 03/22) were provided and signature on this form acknowledges receipt.
