Facility Evaluation Report
Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced Case Management - Incident visit. LPA identified herself to, was greeted by, and explained the purpose of the visit to Skye Hampton, House Manager. LPA also spoke with Administrator Kristen Hollobaugh over the phone .
Today's visit was in response to an Incident Report, which Licensee submitted to the CCLD San Diego Regional Office (received on 10/20/25). According to the report on 10/17/25 Client #1 (C1) was left unsupervised at the facility for approximately 12 minutes. During today’s visit, LPA performed a facility tour, welfare check, and interviewed staff. Staff interviews corroborated the events of the report.
A deficiency was cited per California Code of Regulations (refer to 809-D), Title 22 and an immediate civil penalty of $500 was assessed (refer to the LIC421-IM). A Plan of Correction was jointly developed with administrator.
An exit interview was conducted with House Manager, to whom a copy of this report, the LIC811 Confidential Names List, LIC421-IM, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
