Facility Evaluation Report

Licensing Program Manager (LPM) Sabel Martinez and Licensing Program Analyst (LPA) Eryn Kane conducted an unannounced Plan of Correction (POC) visit. The LPM and LPA introduced themselves and disclosed the purpose of the visit to Administrator Rogelio Hernandez. LPM and LPA stepped away from the facility for a few hours during the visit.

During the visit, the LPM and LPA reviewed records, interviewed staff, and conducted a tour of the facility. POCs for a visit conducted on 2/20/26 were cleared during the visit.

An exit interview was conducted with Administrator Hernandez, to whom a copy of this report and Licensee rights were provided.
