Facility Evaluation Report
Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Rogelio Hernandez, Continual Administrator's Certification expires 04/24/2026. There are currently 68 residents who reside at this home. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, and outdoor areas.. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable.
Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible.
The facility does not have documented required disaster drills. The facility does not have Infection Control Plan. The facility does not have Emergency Disaster Plan. LPA reviewed a sample of resident and staff files. Resident 1 does not have annually updated Needs and Services plan. Resident 2 (over age of 60) does not have required annually updated medical assessment. Staff 1 does not have required hands on training. Staff 2 does not have required TB test. Staff 3 does not have required TB test. Resident 3's medical assessment is not updated and Needs and Services is not updated to reflect current health condition. Resident 3 does not have required Restricted Health Care Plan. Resident 4 not given all prescribed medication. Resident 4's medication not documented on the Centrally Stored medication record. Multiple resident restrooms are not in good repair.
