Facility Evaluation Report

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an a case management visit during an unrelated complaint visit. LPA met with Administrator, Roger Hernandez and explained the reason for the visit.

During the visit, LPA reviewed reporting requirements with Administrator, Hernandez. There were a few incidents that occurred at the facility. Per the Administrator, the incidents were reported directly to LPA and did not use the department email. LPA counseled Administrator in the importance of submitting all incident reports to CCL using the department fax or email.

No violations were issued during today's visit.

An exit interview was conducted with Administrator, Hernandez, and copies of this report and Licensing Rights were provided at the conclusion of the visit.
