The Administrator also stated that the facility offers resources for residents struggling with substance abuse and encourages them to participate in supportive services. The Administrator stated that the facility protocol requires staff to document any incidents of suspected drug use on an incident report log. The Administrator then follows up with the resident’s case manager and connects the resident with appropriate resources. If drug use occurs again, the resident is issued an eviction notice. The Administrator reported that there have been no known incidents of drug use on the premises for some time, and there are currently no evictions related to drug use.

Interviews conducted with residents and staff revealed that no drug use has been witnessed on facility grounds. Staff interviewed stated that the protocol is to notify the Administrator and log the incident in the facility’s report log. Staff are aware that the Administrator is responsible for following up with the resident’s case manager and connecting the resident with appropriate resources.

Based on the evidence in the investigation, the allegation that Staff do not prevent residents from using illegal drugs was deemed unsubstantiated based on the findings. An unsubstantiated finding means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations occurred.

An exit interview was conducted with Roger Hernandez, Administrator. A copy of this report and Licensee's Rights (LIC 9058 03/22) were provided to the Administrator, and his signature on this report confirms receipt of the Licensee Rights.
