This is an official report of an unannounced visit/investigation of a complaint received in our office on

05/26/2022 and conducted by Evaluator Tiffany Holmes

Staff did not protect client from emotional abuse.

Licensing Program Analyst (LPA)Tiffany Holmes contacted the facility to deliver findings for a complaint investigation via tele-virtual. LPA identified herself to, and explained the purpose of the visit and the basic elements of the complaint with Cheryl Baum, Administrator.

LPA Lopez previously conducted interviews with clients, staff, and outside sources, made observations, and reviewed pertinent records. LPA Lopez conducted the initial visit on June 01, 2022 and conducted a tour of the facility.

It was alleged that the staff did not protect client from emotional abuse. Interviews with staff revealed it wasnt a staff that the complaint was about, it was about another client. Interviews revealed that Client 1 (C1) in question no longer lives at the facility and was not able to be interviewed. Interviews revealed both clients lived at the facility and were a couple. Interviews with C2, revealed they were not emotionally abusive to C1. Interviews with C2 revealed that C1 often did not tell the truth and would make up stories. Interviews with staff revealed that C1 made up stories and was untruthful about a lot of topics. Observation of C1s Indivdual Porgram Plan (IPP) last dated 05/11/2021 stated that C1 would decrease incidents of untruthful statements and that they would do some embellishing stories for empathy. This report also stated that C1 is willing to retract an untrue statement when they are confronted about it.

Interviews with outside sources revealed that they did not have anything on record as far as a complaint about a staff or a client being mean to C1 or emotionally abusing them.

The allegation of staff did not protect client from emotional abuse is unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.

An exit interview was conducted with Cheryl Baum, Administrator via face time and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided via email. An electronic email read receipt confirms the documents were received.
